This study helps in disease prevention and control. To that end, epidemiologists used the data to determine specific conditions. One of them is the coronavirus, which is a contagious disease. It first came to China from the seafood market and spread from animal to human and human to human. So, to know the prevalence of COVID-19 in people, most of our researchers, epidemiologists and data analysts used different software for data collection and analysis. One of them is the google form, this is a basic form for data collection. It’s available online. A researcher or anyone can easily collect data and do assignments and quizzes. One of the examples that will help in getting information is how this Google Form works. Consider an example where someone wants to collect data from the general public about coronavirus, scabies, or another disease. Before making a questionnaire, we must find out about the subject on which we want to collect information. Questionnaires are an essential tool that we must first define. Where we are doing a pilot study. Then we put our collected information into a Google form to get information. To create the google form, first go to www.google.com/forms. In this image you see two options, we go with the ‘Go to forms’ button.

Another window will open, where you will see 4 different parts to create a new Google form. We click on the White’ option.

This image shows a different menu where Google Forms asks other questions. Here is your task: The first task has started to put your designed questions into context. Here you see three parts: the questions, the answers and the framework.

Question options:

We will first address the “questions option”. Here we first add the title of our quiz, like “COVID cured or healthy patients”. And add descriptions of our related topics, or you can say, “The purpose of your study”.

In the next 3rd image you see the question and the options provided. Most of the time, the Google form will give you suggestions based on your questions, like Yes and No. or we can add more options by clicking ‘Walk in’

Obligatory: In each question, this option will be given. When you click on it. This option will be active. This will show your participants that the questions need to be answered. So the Google Form will not go to the next section and also not be submitted.

You will see different options in this dialog depending on the requirements of the question. Let’s discuss them one by one.

Short answer: This will help to answer the questions based on participants’ perceptions, or you can say these are open-ended questions.

Paragraph: If your questions are long, you will add this option to your questionnaire.

Checkboxes: These options will help you add your question, as we want to ask about signs and symptoms. We can do with that. For example, I want to ask about signs and symptoms before and after COVID-19. So, I’ll go for it because this option allows my two questions to be combined into one question. This also allows participants to answer questions only once.

Scrolling menu: This also works the same as adding your options.

File download: This will help you upload your questionnaire.

Image: You can add an image here.

We will now discuss adding videos, creating different sections, adding additional questions, and adding text. These options will be displayed just below the questions or on the left side. It depends on your laptop or mobile phone, on which screen you open this “Google form”.
